Date your church was established (first service held)

Date your church was incorporated

Average Sunday morning attendance (not membership)

Yearly total of tithes & offerings

List other yearly income received from daycare, rents, etc.

The amount of loan requested - ($500,000 minimum)

List the purpose of the financing (Purchasing real estate, construction, remodeling, etc.)
